UK. move report extras to fixmystreet.com template
UK specific template was missed in move in 8f2c5942e3a
Also tidy up by removing Lichfield/Reading/Southampton and adding Hart.
(Incidentally fixes commercial ticket mysociety/FixMyStreet-Commercial#525 )
